TECHNICAL AREA
The present invention relates to a preparation for administering mineral-containing preparations for topical application, according to the preamble of independent claim 1 and the therapeutic use of the mineral-containing preparations, preferably in the form of the preparation for the treatment of decubitus in various stages, pressure points of the skin by sitting and Lying and for the regeneration of the skin for example in acne, psoriasis, scar revision, chondrodermatitis, bruises and decubitus.
STATE OF THE ART
The applicant is known from EP 1 339 417 B1 a preparation for the treatment of flat, open wounds, which has a solid active component containing a salt of a biodegradable phosphate and calcium and alkali metal ions. This preparation is based on an amorphously cooled melt of various minerals (hereinafter also referred to as glass ceramic) which are crushed after production, granulated and / or ground to powder form. The mineral preparation has proven to be excellent in the treatment of wounds on surface tissues. The disclosure of EP 1 339 417 B1 relating to the composition and preparation of these preparations is hereby fully incorporated.
The preparation comprises the salt of a biodegradable, ionic phosphate, wherein biodegradable and ionogenic to be understood to mean that it comes to a ionic flux during degradation in use, ie in an aqueous physiological environment. The salt is preferably selected from the group of phosphoric acid, metaphosphoric acid, diphosphoric acid and the counterions of these salts are preferably alkaline earth metal ions. Particularly preferred are calcium metaphosphate (Ca (PO3) 2), beta-tricalcium phosphate Ca3 (PO4) 2, and hydroxyapatite Ca3 (PO4) 3OH. These salts of a biodegradable phosphate can each occur alone or in mixtures. In a further preferred embodiment, the alkali metal ions are sodium and / or potassium ions. These may be added to the formulation in any form from the corresponding halides or oxides, or they may already exist as counterions of the biodegradable phosphates in addition to the alkaline earth metal ions. Particularly preferred are sodium-calcium diphosphates and potassium-calcium diphosphates. In a further preferred embodiment, in addition to calcium, another alkaline earth metal ion, namely magnesium, is present. This may be added as a counterion of the biodegradable phosphates, as a halide salt, or as an oxide of the formulation.
Mineral preparations which have proven to be particularly suitable have the following composition:<tb> P2O5 <SEP> 27.1 to 39.9 mol%<tb> CaO <SEP> 22.5 to 36.0 mol%<tb> M2O <SEP> 14.5 to 30.0 mol%<tb> MgO <SEP> 5.0 to 26 mol% wherein M2O contains 27.1 to 30.0 mol% Na2O and 0 to 12 mol% of K2O.
Mineral preparations which have proven to be particularly preferred have the following composition:<tb> P2O5 <SEP> 32.6 mol%<tb> CaO <SEP> 27.6 mol%<tb> M2O <SEP> 27.6 mol%<tb> MgO <SEP> 12.2 mol%
To prepare the preparation, the starting materials can be melted at 1000-1550 ° C. The material obtained is then comminuted and ground, so that the preparation is produced with a particle size of less than 250 microns. A particle size of 20 to 150 microns is particularly preferred. The preparation is present as granules and / or in the form of finer grains through to powder.
The said preparation according to EP 1 339 417 B1 has the known effect that it stimulates the treated tissue to growth, so that not only a defect can be closed by the new tissue, but also a structure of a tissue structure can be stimulated. This effect of the preparation is disclosed in WO 09 121 616 A of the Applicant, the disclosure of which is hereby also incorporated in full.
The known preparations are applied directly in powder or granular form to the wound or on a - preferably solid - applied carrier. Possible carriers are wax, wax films, wax stencils or silicone carriers. It has already been described that crater-shaped depressions of the wounds are filled in and leveled with the known preparations.
Despite the impressive therapeutic successes in wound healing, there is a need to develop new therapeutic areas and to further improve the administration of the preparations.
PRESENTATION OF THE INVENTION
These objects are achieved by the invention by a preparation for administering a mineral-containing preparation according to claim 1 and the new therapeutic uses of the preparation according to claim 5. Advantageous embodiments and further embodiments are each described in the dependent claims.
A preparation according to the present invention comprises a composition, preferably an aforementioned preparation in the form of a solid mineral-containing active substance in granular and / or powder form, as is known from EP 1 339 417 B1. This composition is also referred to below as glass ceramic. The preparations according to the invention are paste-like preparations or pastes in which the glass-ceramic, ie the powdered or granular mineral-containing preparation, functions not only as an active ingredient but also as a filler. The paste-like consistency and the good adhesive properties of the preparations according to the invention make it possible for the paste to remain on the application site in the preferred topical application, ie after application to the body surface of the patient without further dressing or a separate patch, and adheres as a dry crust after drying.
The preparation comprises an aqueous base, preferably a base comprising a hydrogel and / or a hydrocolloid. The hydrogel and / or hydrocolloid content in the preparation can be up to 80% by weight, with the relative proportion of hydrogel to hydrocolloid being from 0 to 100% and preferably hydroxyethylcellulose forming the aqueous base.
According to preferred embodiments, the preparation comprises 20 to 40 wt.% Solid (glass-ceramic and powdered tricalcium phosphate) and 60 to 80 wt.% Hydrogel and / or hydrocolloid. More preferably, the paste comprises 30% by weight of solid (in the form of glass-ceramic and tricalcium phosphate in equal parts) and 70% by weight of hydroxyethylcellulose.
Preferably, the color of the inventive preparation is determined by the own color of the mineral granules or powder. Its white color, as will be explained below, unexpectedly proved to be extremely advantageous.
After the topical application of the preparation according to the invention dries this and forms a crust-like patch that can remain at the application over a long period of time and protects the treated area from moisture loss. Accordingly, a moist microclimate forms between the skin surface and the outer crust, which promotes the bioavailability of the mineral ions.
Compared to prior art preparations, the preparation according to the invention has a greatly improved adhesive effect, which has proven to be extremely advantageous, especially after drying. After drying on the skin, the preparation is even water-resistant to some degree and does not easily dissolve on contact with water so that it adheres to the treated skin area for up to 72 hours.
Due to the white color of the mineral powder or granules, the paste during application and later the crust-like plaster on the one hand clearly visible, on the other hand, it protects by its absorbing and reflective effect in addition from harmful UV radiation.
It has been shown that the therapeutic effects of mineral Ganulats or powder, respectively the glass ceramic, which were previously detected only in a true aqueous environment, even after drying, respectively after the formation of the crust-like patch stopped. It has been shown that a sufficiently moist microclimate in the skin-facing side of the crust-like plaster is formed by the patch effect already described above, so that the new crust-like patch even after superficial drying on its, the body-facing side still sufficient quantities dissolve dissolved active ingredient respectively on biologically active ions.
The preparations according to the invention are stable pastes, that is to say the glass-ceramic granules or powder do not sediment, even with a longer service life.
After the therapeutic use of the mineral material in the wound healing was already known, it has now surprisingly been found that a preparation comprising the glass ceramic, as a solid active component and 27.1 to 39.9 mol% P205, 22.5 to 36.0 mol% CaO, 5.0 to 30.0 mol% M2O and 5.0 to 26 mol% MgO, wherein M2O contains 27.1 to 30.0 mol% Na2O and 0 to 12 mol% of K2O extremely successful with long-lasting residence time for the topical treatment of acne, especially acne vulgaris and for the treatment of scars (scar revision) can be used.
Preferably, the glass-ceramic is used in the form of the preparation according to the invention, that is to say as a paste with improved adhesive force.
Also in the treatment of chondrodermatitis nodular chronica unexpected treatment successes could be achieved with the new preparation for administering mineral-containing preparations. Chondrodermatitis nodular chronica is a cartilaginous disease in which very painful ulcers can occur at the cartilage margin or the inside of the human ear, which usually requires a generous surgical removal of the affected cartilage area. In Fig. 1, the treatment of a pinna of a patient (Chondrodermatitis nodulare chronica) is shown with the new paste according to the invention.
In the patient before the treatment (Figure 1A) with the new paste has already tried unsuccessfully to treat the defect with a skin graft. However, the chronic inflammatory process continued. This progression of the inflammation could be stopped by applying the paste as shown in Fig. 1B. In Fig. 1B is clearly seen that the new preparation is dried out after topical application to the ear to a crust-like patch. In this form, it could remain on the treated ear for several days.
From Fig. 1C it can be seen that in addition a remodeling of the skin defect was achieved by means of the treatment with the new paste. In addition, anterior wrinkle reduction could be achieved, which corresponds to tissue regeneration.
In the treatment of acne with the novel preparation according to the invention in paste form very good treatment results were also achieved when the preparation could dry out after topical application to the crust-like patch and remained on the treated skin.
With the inventive preparation for administering mineral-containing preparations were also achieved in the treatment of pressure ulcers at various stages, in the treatment of pressure sores of the skin by sitting and lying and in the regeneration of the skin, for example in psoriasis successes. Again, preferably the mineral-containing glass ceramic is used in the form of the preparation according to the invention.
The inventive glass-ceramic is neither sintered nor porous and not rough. It consists of dense, compact smallest particles of the quality of granules, powder or powder, which are made by grinding from a cooled melt. The mode of action of these compact particles of granules / powder is not based on absorbency, absorption or absorption, as in other applications of the prior art, as in the case of porous materials, but presumably on a positive influence on the cell metabolism of the target tissues, a so-called energetic Charging the cells by the ionic flux (from the salts) into the cell germs and cells. This achieves stabilization of the cell metabolism and of the cell walls, which in turn contributes to an improved defense against proteolytic bacteria (pseudomonads, inter alia). On the one hand, the glass ceramic thus stabilizes the physiological cell metabolism, on the other hand, it can also act bactericidal and fungicidal. Likewise, pollutants such as free radicals or other pollutants could also be bound or neutralized by ion influx. These assumptions about the mode of action are based on experience from the clinical applications of different materials. However, they should in no way be represented as secured or understood as a restriction to these principles of action.
According to the present invention, the granules and / or powder of glass-ceramic additional tricalcium phosphate is mixed and this mixture is formulated with hydroxyethyl cellulose as a paste. It should be clearly distinguished here between the calcium phosphate in the glass-ceramic, which is present in amorphous form, and the additional powdery crystalline tricalcium phosphate, which is additionally added.
15% by weight of the glass-ceramic granules or powder with 15% by weight of tricalcium phosphate powder and 70% by weight of hydrogel, preferably hydroxyethylcellulose, are mixed and formulated into a paste.
The aforementioned paste has a greatly increased adhesion to the skin of the patient and is even waterproof to a certain extent.
A further advantageous therapeutic use of the paste according to the invention lies in the treatment of burn wounds.
It could also be shown that the new paste according to the present invention shows good results in the regeneration of the mucosa and in the treatment of abscesses in the oral cavity.

A preparation for administering a mineral-containing preparation for topical application comprising 10 to 20% by weight of a mineral preparation, the preparation having a mineral granulate or powder as solid active component which contains 27.1 to 39.9 mol% P205, 22.5 to 36.0 mol% CaO, 5 to 30.0 mol% M2O and 5.0 to 26 mol% MgO, wherein M2O 27.1 to 30.0 mol% Na2O and 0 to 12 mol% of K2O contains, characterized in that the preparation 10 to 20 wt.% Tricalcium phosphate preferably as a powder and 60 to 80 wt.% Hydrogel and / or hydrocolloid, wherein the relative proportion of hydrogel to hydrocolloid can be from 0 to 100%.
